This may be controversial, and certainly radical but, as with every self-inflicted or wholly avoidable incident (eg rock climbing, base jumping etc) then the NHS should present the patient with a card payment machine and demand payment prior to treatment.

Now my above comment was made 'tongue in cheek' to a certain extent, but my point is this - why should we foot the bill (via the taxation system) for somebody to get free treatment when they've willingly gone out and got totally wasted drinking alcohol. Similarly for people partaking in dangerous sports, to my mind they should be made to take out insurance cover that would foot the bill should anything happen.
